Your initial appointment will consist of an examination performed by Dr. McCormick. At this time, any necessary radiographs (x-rays) will be taken; a visual exam, oral cancer exam and periodontal charting will be performed. A thorough review of your medical history is also an important part of the complete examination. After all the diagnostic tests have been performed and evaluated, a consultation with Dr. McCormick will take place. Your questions and concerns will be addressed at this time.
When you call our office to reserve time for your first visit, please assist us by providing the following information:
- All demographic data
- Your dental insurance information
- Information regarding your most recent dental xrays
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the first visit.
Please alert the office if you have a medical condition that may be of concern prior to dental treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
